In 2019, the FDA granted approval for SPRAVATO®, an esketamine-derived nasal spray designed for adults grappling with Treatment Resistant Depression and suicidal ideation. Esketamine plus an oral antidepressant has been found to relieve the symptoms of depression within hours, rather than the usual weeks most antidepressants can take.
Spravato is an N-methyl-D-aspartate (NMDA) receptor antagonist that is believed to work differently by action on a pathway in the brain where glutamate, a brain chemical, works with other brain chemicals to balance well-being and depression. By blocking the NMDA receptor, esketamine leads the formation of new neural connections and the restoration of neural plasticity.

Where is Spravato Available? *Spravato is only available through a restricted program called the Spravato REMS (Risk Evaluation and Mitigation Strategy). It is administered intranasally while under clinical supervision in a REMS certified Spravato treatment center. Patients can only receive treatment from one REMS certified Spravato treatment center.
Taking SPRAVATO® nasal spray is a straightforward process, conducted under the supervision of our healthcare professionals in a comfortable healthcare setting. Here’s what you can expect:
Your journey at Hopemark Health begins with an evaluation with one of our psychiatric medical providers. During this appointment the provider will inquire about your medical and mental health treatment history including medications, treatments and symptoms. If you meet the criteria for Spravato the provider will recommend you for treatment. Once a complete history is gathered, your provider will lay out the options, and together you will create the plan to move forward.
Insurance covers Spravato in most cases but does require pre-authorization from your insurance carrier prior to beginning treatment. This process can take time to complete.
Throughout your SPRAVATO® session, our healthcare team will closely monitor your well-being. Patients are monitored with state-of-the-art Caretaker System to see real time blood pressure, heart rate, and oxygen saturation measurements. We’ll assess your readiness to leave the healthcare setting after each use.
To minimize the risk of side effects, it’s crucial to follow specific guidelines, such as refraining from eating for at least four hours before using SPRAVATO® and avoiding liquids for at least 30 minutes prior. You will discuss these guidelines with your provider prior to your appointment.
If you’re taking nasal corticosteroids or nasal decongestants, ensure you take these medications at least one hour before using SPRAVATO® to prevent potential interactions. Please discuss with your provider prior to your appointment.
You can not drive or operate machinery immediately after so it is important to arrange for a ride home after your SPRAVATO® treatment.
Patients are observed and monitored by our expert nursing staff using state-of-the-art Caretaker Systems to see real time blood pressure, heart rate, and oxygen saturation measurements. You will be in a private room with the options to relax with an eye mask and listen to music and / or a TV when desired.

You will be ready to go home after the medical staff clears you for release. You cannot drive home, so please secure a ride with a friend or family member.
